Helper Placemat

The Senior Memory Helper Placemat helps seniors or others with
memory problems due to Alzheimers, senility, brain trauma or other
factors to recognize people who come in contact with them. These can
be family members, caretakers, clergy, neighbors and others, who may
visit or phone them. With the placemat always nearby the patient can
refer to the placemat to help understand the person’s association to
them. Placemats are laminated for continuous use.
To order send up to 6 jpeg photos as attachments in an email to:

For an additional $15.00 charge you may send photos for us to scan
onto your placemats.
Your original photos will be returned to you with the placemat.
Please number each photo. Include the name and association of each
person by photo number (e.g.) #3 Mary Ann Sampson, neighbor).
Here are three ways to order:

- If mailing prints to scan, you can send a check in the
amount of $50.00 (includes scanning charge for photos) plus
$6.00 S&H (AZ residents add 8.1% retail sales tax).
Checks made payable to Its Time To Tell Your Story.

- If you are mailing a CD of jpegs rather than prints the
price is $35.00 plus $6.00 S&H (AZ residents add 8.1% retail
sales tax.)
